Terms and Conditions - Electronic Consent

  1. The Electronic Consent programme is operated by Green Cross Health on behalf of Life Pharmacy and Unichem Pharmacy.
  2. Each participant of the Electronic Consent programme is bound to the full terms and conditions of the programme, as varied from time to time.
  3. Green Cross Health may cancel, withdraw or alter the programme, including these terms and conditions at any time at our discretion.
  4. Green Cross Health may refuse participation in the Electronic Consent programme at any time in its absolute discretion.
  5. By competing the Electronic consent form, you are granting your consent to Green Cross Health for the provision of the services as detailed in the Electronic Consent programme.
  6. Green Cross Health will only use the data it collects as part of the Electronic Consent programme for the purpose it was intended.
  7. Green Cross Health reserves the right to notify law enforcement agencies and to pursue any remedies that may be available in the case of improper use or fraudulent activity in relation to the Electronic Consent programme or any other systems.
  8. If you have opted-in for communications, Green Cross Health may from time-to-time send you offers or promotions relevant to the services being provided under the Electronic Consent programme.
  9. The Electronic Consent programme is compliant with the Privacy Act 1993 - New Zealand.
